Managing you diabetes is imperative to managing your overall health status. Diabetes plays a significant role in the systemic regulation of the body processes. Managing blood sugar is the key to living well with diabetes and eating well, while managing an adequate diet, it provides a foundation for appropriately management of blood sugar.
The diet needs to comprise itself with a balance of proteins, fats, and carbs. The most appropriate way to manage this, is to control the amount of carbs that are consumed and limit the amount of simple sugars. Recommended food groups that should be included in the diabetic diet should be vegetables, fruits, grains, protein (fish, lean meat, eggs, nuts and beans), dairy-nonfat or low fat. Avoid high fructose, junk food, wheat breads, and food items that can break down to become sugars. It is important to effectively manage the administration of medications with diabetes mellitus. Insulin needs to be closely monitored, understood on its intended effects and knowing the differences between the types of insulin and how they affect the blood glucose and the time that effects take place. Adequate amounts of physical activity is needed to promote a healthy lifestyle. Simple walks, non-sedentary activities will help lower the effects of diabetes. If there is not appropriate management of the blood glucose within the body, it can cause devastating effects to the body, and can ultimately lead to further dysfunction of distribution of insulin, it can affect the kidney function and lead to failure of other organ systems. Dehydration can be caused by a multitude of reason which includes diarrhea, vomiting, sweating in excess, urination in excess fevers and burns. People who are most at risk of developing dehydration are the elderly, the young and those who suffer from chronic illnesses such as diabetes. Diabetes causes an excessive elimination of fluid and waste due to the increased need to urinate. It is important to stay hydrated because of the effects of dehydration can produce everlasting effects on the functionality of the kidneys and cognition. Dehydration can be mild, or it can be severe enough to be life threatening. If dehydration worsens, it can present as confusion, fainting, raid heart rate, rapid breathing, lack of urination and can lead to shock or even death. (National Institutes of Health, 2019).
Management of Diabetes: (Condition, Potential Environment and Lifestyle Changes)
Tina, it is important to monitor the healing of your wound on the right foot. The wound and the infection that exist inside has breached multiple layers of skin. The wound poses an increased risk of a further extreme of the infection. The infection can extend out from the injury and spread throughout the extremity or become systemic throughout the entire body. Keep the wound clean of any dirt, contaminants, or contagions. It is vital to keep the wound dry, because if the wound remains wet or saturated, it can grow bacteria which can result in worsening infection.
Management of your diabetes, ensuring a better diet which should include high amount of protein will lead to reconstruction and healing of the wound. Antibiotics are crucial in eliminating the infection and the bad bacteria. Even when the infection appears to be gone, it is important to continue to take the antibiotics for the whole time that they are prescribed. If you stop the abruptly, it can cause adverse effects that may not be associated with the wound.
Maintaining the diabetes and the blood glucose levels are important to properly regulate the organ systems that are involved. Proper treatment is essential in living a healthy lifestyle and limiting adverse effects that can worsen effects on the body. Increased physical activity is important to maintain proper body functions. Physical activity can help regulate the insulin and diabetes issues that can be met. Wound Care and Techniques
The management of diabetic foot ulcers requires offloading of the wound by using appropriate therapeutic footwear, daily saline irrigation or similar dressing that will provide a moist wound environment. There may be debridement if the wound worsens in its condition and continuing antibiotic therapy. To promote effective treatment and wound healing, optimal control of blood glucose levels and evaluation is vital to the efficacy of the treatment. (Rowe, 2020). The fundamentals of good care for wound begins with adequate debridement, offloading, moist wound care, and the revascularization of the wound. The circulation has to remain flowing to the wound in order for it to properly heal. When cleaning the wound, there is a need to use a aseptic technique, (bacteria free/clean process). It is recommended to use a foam dressing which will be absorbent and prevent drainage. Keep the dressing clean, dry and if there is drainage present, it may be time to change the dressing. It is recommended to irrigate with sterile water or normal saline to wash away contaminants. Keep the extremity elevated and avoid any direct pressure. (Rowe, 2020).
Medication Education
Albuterol works by dilating the bronchioles which will make it easy to breathe. It is a short acting agent that will take effect shortly. It can cause slight agitation or a faster than normal heart rate, headaches, nausea or dizziness. Albuterol may increase the blood pressure. (Schwartz, 2019). Insulin glargine works as a long acting insulin and will provide insulin coverage over high blood glucose levels for 24 hours. There is no peak and it is normally administered at night. Side effects can include chills, blurred vision, fast heart rate, weakness, tingling of feet and fingers.
These are all symptoms of hypoglycemia. Metformin is an oral diabetic medication that can control the glucose levels. Much of the time, medications such as metformin, can control elevated sugar levels with the use of excessive amounts of insulin. Metformin can cause diarrhea and lead to dehydration, so it important to stay hydrated. Metformin can also cause weakness, muscle pain, low B12 levels, upper respiratory infections. Linezolid is used to control and treat bacterial infections present within the wound. It is important to take medication as long as it is prescribed. Side effects can include nausea, vomiting, diarrhea, insomnia, discolored tongue and oral thrush. It is important to wash out your mouth as much as possible. (Ogbru, 2017). When should the doctor be contact or when should you go back to the ED?
It is important to recognize when your condition worsens. It is also important to recognize the symptoms that will allow you to know something is wrong. The symptoms that indicate that the infection is worsening is a marked fever of >100.4, chills, worsening pain or any new inflammation that can be found by redness or swelling. If these are seen do not hesitate to return back to the ER and seek attention from your physician. Monitor the hypoglycemia and the effects of the diabetes. Looks for changes in mentation such as sudden loss of consciousness, confusion, sweating, agitation. When this happens have some fruit juices, eat candy or take glucose tablets. If uncontrolled call the doctor or return to the ED.

Activity 5 Comprehensive Physical Assessment
This Comprehensive Assessment activity provides the opportunity to plan and conduct a full health assessment on a patient in a single clinic visit. Students spend, on average, three hours on this activity.
Before entering the clinic room you will complete the Plan My Exam activity to organize the exam procedures you will use into a sequence that moves smoothly from head to toe by mapping them to the region of the body where you will perform them. After you have planned your exam, you will enter the clinic room and conduct a Comprehensive Assessment by interviewing and examining your Digital Standardized Patient.
These prompts help you think more deeply about your performance in the activity. Reflective writing develops your clinical reasoning skills as you grow and improve as a clinician and gives your instructor insight into your learning process. The more detail and depth you provide in your responses, the more you will benefit from this activity.
